a clear copy of final 6th grade report card

a clear copy of final 7th grade report card

a clear copy of your most recent 8th grade report card

PDF copies of NY State ELA and Math scores for grades six and seven

a clear copy of your most recent complete tuition bill from your currnent school (specifically for students attending tuition charging schools)

request that a school administrator or teacher complete and submit the Recommendation Form *visit: www.stjean.org to get the link
